For over 40 years, we’ve been the first place many Chicagoans experience care without judgment.
You deserve care that sees all of you. At First Point Community, we’re here for your health, your mental wellbeing, and the everyday needs that don’t always fit neatly into a doctor’s visit. We offer comprehensive and compassionate care, without judgment.
Health Services
- Primary Medical Care - Acute and chronic disease management, preventive screenings, and HIV/AIDS prevention and care
- Dental Care - Preventive care, restorative treatment, and emergency relief
- Behavioral Health - Trauma-informed therapy, mental health support groups
- Substance Use Treatment - Medication Assisted Treatment (MAT) and recovery support
- Psychiatry - Acute and chronic serious mental illness, psychological disease management, and psychiatric medicine management
Support Services
- Personal Care Center - Showers, laundry services, and hygiene kits
- Housing Assistance - Assistance applying for Chicago's housing waitlist Coordinated Entry System (CES), and referrals to other housing resources
- Transportation Services - Rideshare and transit assistance for appointments
- Benefits Enrollment - Assistance with Medicaid and applying for benefits and entitlements
- Language Services - Culturally competent care in 90+ languages
Nutrition Services
- Community Market - Access to nutrient dense food options to promote a more healthful diet
- Nutrition Services - Dietitian-led guidance on meal and food selection from current grocery centers. Referral for Medical Nutrition Therapy (in-depth nutrition assessment with personalized diet plans and counseling). Group nutrition education and food demonstrations
- Food Assistance Enrollment - Assistance with Supplemental Nutrition Assistance Program (SNAP) applications
HIV/AIDS Care
First Point Community provides specialized care and support for patients living with HIV/AIDS, including:
- Testing
- Prevention
- PrEP
- Grocery Centers
- Guidance for medically tailored meals
- Ongoing care
Refugee Services
If you are a refugee looking for mental health support, we may be able to help.
To get started, you’ll need to meet the following three requirements:
1 – You are Office of Refugee Resettlement (ORR) eligible as a Refugee, Asylee, SIV, VOT, or Humanitarian Parolee
2 – You feel you need support for mental health symptoms
3 – You currently have, or qualify for Medicaid
